Quilting the Tummy Tuck?

For decades, a standard tummy tuck involved lifting up the loose abdominal skin, repairing relaxed muscles, then pulling down and cutting off the extra skin. The analogy is much like pulling a sheet tight on a bed and cutting off the extra. The tightened skin was left to stick to the abdominal wall with gentle pressure from wearing a compressive binder as the patient healed. A drain was placed so that fluid would not accumulate under the healing skin and keep it from sticking to the abdominal wall. Fluid collections are known as seromas. Recently two surgeons, brothers, in the Midwest began sewing the… Read More

How many drains? One, Two or none?

Drains are tubes that are left under the skin during an operation. They are used to remove pockets of fluid that can accumulate. After any surgery, the tissues ooze fluid to a certain degree as they heal. Fluid collections during healing can sometimes lead to infection. Despite this some areas are more prone to collections than others and not all surgeries need drains. Usually drains are removed within a week or so when there is little fluid coming out. The fluid enters a small clear bulb that looks like a grenade. The idea of removing a drain scares patients, but it is more… Read More

A recurring problem with liposuction.

Over the past few years I have seen an increasingly common pattern in patients coming in for abdominal enhancement. I am writing about it now in this blog, hoping you will read it and be able to avoid the same problem. The scenario: a woman will come in concerned about a liposuction that was done elsewhere. When I examine the patient I find a wavy irregular lower abdomen over liposuctioned, and a full under suctioned upper abdomen. Bulgy top, wavy Bottom. The conversation goes something like this: So you had liposuction? Yes, it looked like a good deal. You had your liposuction… Read More

Voluma so far…

Allergan, the company that makes Botox, breast implants and Juviderm products is now heavily pushing its latest filler Voluma. I see it marketed on TV all the time before the evening news. From the commercials it really is not clear to me that their beautiful models have had treatment with Voluma, but boy they sure are very attractive. For those of you who know me, and I hope if your reading this by now you at least feel like you know me, I am skeptical of new things. I don’t jump on bandwagons easily, but so far I really like… Read More

Arm liposuction 50/50?

Arm liposuction 50/50? A patient saw me in the office yesterday. She is having a tummy tuck and breast reduction this fall. She came in again to discuss adding liposuction of the arms at the same time. She was confused though. She had done her usual research on the internet; technologies guide to cosmetic surgery, and found that only about half of the people who had the procedure in the U.S. were happy with their results. Why? She asked. I explained….the key to arm liposuction is not the fat, but the skin. We can suck fat out of almost anywhere,… Read More

Ultrasonolaserliposuction with Power Boost!

Ok, I made up this name. There is no such thing as “Ultrasonolaserliposuction with Power Boost”, but I wouldn’t be surprised if I saw it advertised somewhere. I really just wanted to capture your attention. I have found that anything associated with the word laser, ultra or the like tends to attract attention. So why am I talking about this? Because a cool device name doesn’t always translate into the right tool for the job. Laser liposuction, Ultrasonic liposuction, Power Assisted liposuction and even good old regular liposuction are all excellent tools. They all remove fat well, but they all shrink up skin poorly, despite… Read More

Hair Thickness- Just when you think you’ve heard it all!

I’ve been in practice now for almost 20 years. Over that time I have fielded thousands of questions, to the point that I get surprised when I come across a question I have not already answered or pondered before. I met with a gentleman yesterday considering hair transplant for male pattern baldness. He had seen other doctors and had done a good deal of research. I like it when patients have done their own research, it makes going over details and our own approach easier. We spoke about strip graft versus follicular unit extraction and the robot. We discussed a day of the… Read More

Hair Transplant: FUE or Strip Graft?

Traditionally, hair harvested for transplant has been removed as a strip from the back of the scalp, cut into individual hair shafts under a microscope and transplanted to the front of the head or other areas where the hair is missing. The area where the strip is harvested is then closed. The newest tool in hair transplant is FUE or Follicular Unit Extraction. This tool harvests one hair follicle at a time leaving a small puncture to heal in. Each technology has pros and cons and yet each has a place. While strip excision leaves a scar it is generally… Read More

Dr. Tattelbaum Discussion on Breast Implant Techniques

An Overview of Breast Implant Techniques For this article I would like to go beyond the basic facts that are posted all over the internet and give a personal opinion based on almost two decades of experience and thousands of breast implant procedures performed. This blog covers the most frequently asked breast implant questions: 1)      Under or over the muscle: I generally favor implants under the muscle because they tend to look more natural.  This is because the muscle gives a gentle slope to the upper breast. When the breast implant lies under the muscle mammograms are easier to interpret. Infection… Read More
